Artist Information
In 2011, ADA is building a more efficient database system to assist us in our record-keeping. Please take a few minutes to review the form at left, and gather the information that you will need to complete the form in one sitting.
You will be asked to provide general contact information as well as detailed information and/or scanned images regarding:
- air and ground transportation travel preferences
- passport validity
- driver’s license
- auto insurance
- housing preferences
- allergies
- who to contact in an emergency
- your AGMA membership status
- frequent traveler/affinity programs
You will also be asked to fill out and complete a 2011 W9 tax form that we can keep on file so you are paid in a timely fashion by your sponsoring organizations. You may download and complete the form by clicking here, but then you must sign and scan a hard copy of the form, so that you can upload it as a .pdf file for our records.
Again, please take the time to gather this information and scan the appropriate documents so that you can complete the form in its entirety in one sitting.
